Dana Dickson an RN and Clinical Liaison with Gentiva was in for our Senior Spotlight this week, to talk about Gentiva's 2013 initiative to reducing re-hospitalization called the Standards of Care.
Dana talked to us about two of the key elements to reducing rehospitalization, physician follow up and medication management.
