Lori Newcomb, RN, care manager at EMMC family practices joined Carolyn Callahan on TV5 News at Noon to talk about talked about Bangor Beacon which is an exciting opportunity for our region and for our nation.
We are one of 17 communities in the country taking part in a national grant to help improve the care of our sickest and highest cost patients. Bangor Beacon is a combined effort between EMMC, St Joe's, and Penobscot Community Health Care. These partners are working together rather than competing to benefit people with chronic diseases. We are demonstrating that the use of a care manager in conjunction with a primary care doctor improves the lives of people with four chronic diseases. Those are COPD, congestive heart failure, diabetes and asthma. The grant is also allowing us to improve our abilities to use technology to benefit our patients through electronic medical records.
